Assist Life Stage/ Transition
Transitions such as moving out of one’s home, joining the university, or getting a job are part of everyone’s life. It is never an easy task both emotionally and practically, and it becomes a little more difficult for people with disabilities. Change can be hard and with it comes new environments and settings.
The National Disability Insurance Scheme (NDIS) offers a wide range of support to assist life stage transitions. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the person. Disability support agencies offer short-term and long-term support options and life transition planning services depending on the needs of the customer. Under the NDIS assist life stage transition, they enable a friction-free move and encourage the participant to relocate the person in a new setting and create a comfortable ethos – accommodation, social connections, finances, and recreation for them to feel at ease.
Our Life Stage Transition program includes short- and long-term support that focuses on assistance to live at home, community participation, and helping you to coordinate your own support. This includes:
- Support connection and coordination
- Capacity building skills
- Development of independent self-care and living skills
- Crisis and conflict resolution
- Establish supportive networks in the participant’s communal setup
- Linkage support to other supportive community services and programs
